The main role of clinical decision support systems (CDSS) is to improve patient safety. This is done by reducing errors such as misdiagnosis, medication errors, or adverse drug interactions.

· Impact on clinician autonomy

CDSS can have an impact on clinician autonomy by continuously suggesting plan of care, diagnostic testing, and prescribed medications. Many clinicians or physicians can see this as a threat.

· Adherence to clinical guidelines

CDSS allow for physicians to adhere to clinical guidelines that are provided by evidence-based practices. This helps improve overall patient care and patient outcomes.

CDSS in itself is a very expensive system to incorporate into electronic health records. This can impact the implementation of the system itself.

CDSS also improve cost for both the patient and the hospital by decreasing the length of stay, suggesting cheaper medication alternatives, or reducing duplication of diagnostic testing. (Sutton, et. al, 2020).

· Alert fatigue and inappropriate alerts

Unfortunately, there is always falls to each system. Studies have shown that 95% of alerts are inconsequential and many physicians do not trust or disagree with the alert. This can lead to dismissal of the alarm and/or not reading the alert.

Working as a Family Nurse Practitioner (FNP), I will see a variety of people ranging anywhere from infants to adults. Clinical Decision Support Systems (CDSS) will be extremely helpful in diagnosing, prescribing, and orders tests. For example, let’s say I have a patient who comes in pain that feels like burning in his feet. This patient has a known history of chronic kidney disease, diabetes, hypertension, and chronic obstructive pulmonary disease. I diagnose the patient with neuropathy and want to prescribe Gabapentin for pain relief. I take into consideration that Gabapentin has effect on the respiratory system, but when prescribing forget to dose the medication based on his renal disease. CDSS will alert me to adjust the medication to an appropriate dose for the patient and recommend follow up treatment.
